1. High levels of an enzyme called 5-alpha-reductase occur in cells of the hair follicle.
2. This enzyme converts testosterone into DHT.
3. DHT causes the robust terminal hairs to miniaturize.
4. This leaves soft, fine hairs that provide poor scalp coverage.
5. The growth phase becomes shorter until these soft hairs are lost for good.

Excessive hair loss can occur at any age and may even stop only to start again. As time progresses the process of hair loss accelerates leading to its logical conclusion.

Understanding this process helps to explain how the currently available treatments for male pattern baldness work. Propecia is one of only two FDA-approved hair loss treatments and it works by interfering with the process outlined above. In brief, Propecia inhibits the formation of DHT in the system thus slowing down and eventually stopping the adverse effects of DHT on the hair follicle.

Genetic traits make hair follicles shrink, resulting in the actual density, or thickness of hair grown diminishing, so although the follicles are still alive, they are no longer able to perform their task. With female pattern baldness, unlike the male equivalent, it is not so readily accepted socially and in many cases it can have devastating effect on the sufferers emotional state and confidence. It is often linked to hormonal changes, following events such as the menopause, or as a result of stopping, or starting oral contraceptive pills. Childbirth is also a known factor.

Hormone levels increase as the pregnancy begins and slows down the hair growth cycle. Hairs that should stop growing continue to grow beyond their usual life cycle. Often this means that the hair appears to grow thicker as more hairs are present than normal. Other causes can be numerous and include stress, restriction of the blood supply, a poor nervous system and hormonal influences. With female pattern baldness, the hair loss is generally more uniform over the scalp than in the male counterpart, but also results from a complex chemical reaction when the enzyme 5-alpha-reductase converts the testosterone in the system into DHT or dihydrotestosterone. Female pattern baldness affects approximately one-third of all susceptible women, whereas the male strain appears to be slightly more common, affecting around 40 per cent.

Medical science does not at present know exactly why some men with high testosterone levels (marked by heavy beards, an excess of body hair and deepness of voice) do not succumb to male pattern baldness, while others, often with lower testosterone levels, do. Nor is it understood why it only affects the hair follicles on top of the head and not those on the back and sides. Scientific updates however, are constantly being issued and there is optimism that a positive conclusion will eventually be drawn."

Many men therefore assume that the higher dose is more effective in preventing the conversion of testosterone into DHT. The extent to which this is true is debatable although the basic facts do tell us Proscar blocks slightly more DHT formation. Some physicians feel its use is warranted in a small proportion of older men and women who previously have not responded to treatment with Propecia.

3. Tretinoin (Retin-A) - this is well known for the treatment of acne but medical studies have shown it also helps stimulate hair regrowth. Some experts argue that tretinoin enhances the absorption and penetration of minoxidil into the hair follicles. As a result, several formulas of tretinoin and minoxidil are available, generally only by prescription.

Treatment of Female Pattern Hair Loss with Oral Antiandrogens

Sinclair R, MD, at British Journal of Dermatology has written the Treatment of Female Pattern Hair Loss with Oral Antiandrogens article and writes "The authors present an open intervention study to assess if antiandrogen therapy is effective in the treatment of female pattern hair loss.

Eighty women were entered into the study, all of whom underwent clinical evaluation and biopsy of the central mid scalp to provide both clinical and histologic evidence of female pattern hair loss. Patients also had hematologic screening to exclude systemic lupus erythematosus, thyroid disease, zinc and iron deficiency, and hormone dysregulation.

Patients were then divided into 2 treatment arms. Forty women received spironolactone 200 mg daily. The other 40 women received cyproterone acetate, the dose of which was determined by the presence or absence of menopause. Twenty-two post-menopausal women received 50 mg daily, while 18 pre-menopausal women received 100 mg daily for 10 days of every month in combination with oral contraceptive pills. Sixteen months was the average treatment duration. Patients were not allowed to use topical minoxidil. At baseline and then every 6 months, photography of the frontal and vertex scalp was performed on each patient. Assessment of these photographs was completed by 3 clinicians, blinded as to patient information and to treatment regimen, using a rating scale of increased hair density, decreased hair density, or no change.

The frontal and vertex areas of the scalp were given independent ratings. Photographic assessment served as the primary outcome for efficacy. Clinical grading of the mid scalp using a standard scale was also done. Analysis of the data showed no difference between the 2 treatment arms so the authors combined the results of both arms. Forty-four percent of women had increased hair density in either the frontal or vertex areas, or both. Another 44% showed no increase or decrease in hair density.

Twelve percent showed decreased hair density in one or both areas. Variables such as menopausal status, family history of hair loss, thyroid function, duration of hair loss, and patient age showed no statistically significant difference regarding treatment response. However, women with advanced hair loss of the mid scalp and hence a mid scalp clinical grade of 3 or 4 did show a statistically significant difference in regards to response compared with those of lesser grades. Adverse effects of the 2 treatments were not discussed in the article."

Do note that Rogaine does not really cure baldness; as a matter of fact, no studies show how it works. It does not grow hair on a completely bald scalp; it tends to delay hairloss in such areas that are extremely reduced. It is possible that Rogaine extend the growth stage of the hair, which ceases or delay the process of miniaturization. Rogaine should be applied twice daily; once a day treatment is not effective, and hair growth results may not be as excellent as you imagine; since Rogaine works by increasing hair thickness, that are miniaturized already, many individuals see noticeable growth of fuzzy and fine hair which does not likely to grow long. Usually it is effective only top of the head or the crown, and not at the front area of the head. It normally takes six to twelve months to see some results; to some individuals, they do not even notice a change until they stop using the medication. Do note however, that discontinuing the medication for as long as three months, any regrowth or hair appearance will disappear. So then if Rogaine works for you, you should continue with the medication or you will lose any benefit that it has shown.

The drug finasteride, available in the market as Proscar for treatment symptoms of prostate enlargement, perform by restraining the action of the enzyme 5-alpha-reductase, the enzyme in charge for changing testosterone to dihydrotestosterone (DHT). As DHT is responsible for hair growth also in the face, increased occurrence of acne, enlargement of the prostate gland, and is essential in the advancementt of male pattern baldness (androgenetic alopecia). Testosterone, is the male hormone, and is in charge for the changes that take place during puberty such as the lowering of one's voice, an increase in muscle mass, etc. When testosterone is not enough, there is a possibility that sex drive will decrease depression, erectile dysfunction, and a loss of muscle mass. Meaning, most of what DHT carry out, an individual can do without. However, Testosterone is very essential. In studies, men who took the one milligram dose of Propecia, their levels declined by about two-thirds; testosterone levels were maintained within normal range, and also a ten percent increase was noted.

Hair Restoration In The Young Patient

We do not have a crystal ball and we cannot predict your future. If you begin surgery when you are young, you may find your hair loss exceeds your donor capacity. At this point you may wish that you had not begun doing surgery and simply desire to return to your natural state of baldness. Unfortunately, I find many individuals present to my office with this very complaint. They simply want to return to a natural, bald state.

The younger you are when you first experience hair loss, the more hair loss you can expect to occur over the remainder of your life. Like it or not, hair loss is a life long process that will not cease until you die or we discover a medical cure for hair loss. We cannot predict when we will see a cure for hair loss. If you are already an advanced degree of hair loss in your early twenties, I would recommend against hair restoration surgery. If you degree of loss is less, and you have excellent hair characteristics, we might consider a surgical solution at some point.

I do not believe that any young individual should begin surgery in his early twenties without first trying medical treatment. Most medical treatments such as minoxidil, propecia, avodart, and topical spironolactone do not grow a significant amount of hair in most individuals. What they do often though, is slow or even stop hair loss. This is very important to you. You are buying time until we have a better solution and perhaps limiting the amount of hair restoration you will require over the course of you lifetime. This may ultimately save you money and preserve you coverage for a longer period of time. You may also slow your hair loss until such time that you become more comfortable with your degree of hair loss. This means that if your donor supplies will allow us to give you a full look in the front, but a thin or bare look in the back, vertex, you may be happy with this result at age 50, but very disappointed at age 23. In other words, people tend to become more comfortable with their hair loss as they age. This is not an absolute rule though. I have seen many men in their 50s, 60s, 70s, and 80s who were not happy with their hair loss and sought treatment.

If you do consider surgery at a young age, I recommend FIT. This is the only true stand alone procedure. In other words, you can elect to shave your head down the road and still appear natural because you will not have a linear strip scar in your donor region. You must realize that this is a new procedure though, and there are many things about it that we are still unravelling. We know that in due course we are simply going to get better and that the results should continuously improve. This does not mean that we are disappointed with the results to date. Rather, we find the results are fantastic. We do expect major improvements over time and a better definition of the complications.

I do not recommend strip incisions in the young patient and feel that it is imporatant for any physician to disclose the potential beneifits of FIT/FUE and the linear scar complication to any young male seeking hair restoration surgery.

FIT also allows you to pursue the "less is more" illusion of coverage. Here the hair is cut very short in a crew cut fashion. The balding or thinning areas are restored so that they appear thin but natural. We do not attempt to restore the density you were born with, but give you a thin look. When the hair is cut short, we find the illusion is such that you appear to have better coverage than you really do. In other words, less coverage give you a more full illusion.

Typically, I recommend that any person delay hair restoration surgery until they are 23 years of age. I would consider FIT placed in a conservative fashion in a younger male though, but only if they were fully educated and had been counselled by those who experienced hair transplant surgery at a young age, but now spend a large portion of their time educating others about their unfortunate mistakes. One might consider speaking with real hair loss patients. The longer you can delay your procedure, the better when you are in your early twenties."

3. Photochemical destruction: This is photochemical destruction by use of laser light. A chemical reaction is induced in the light sensitive material. These chemicals reach with hair follicles and cause chemical damage to the hair shaft and hair follicle structures.

In simple words, the procedure involves:

1. A hand-held laser is pressed against the skin and is activated for a fraction of a second through an automatic switch.

2. The laser light passes through the skin's surface and reaches the hair follicles, which include the hair shaft, hair bulb and hair follicle matrix. The melanin present in these structures is highly laser-sensitive.

3. The laser destroys the hair bulb and hair follicles.

4. A carbon lotion, depending on the laser wavelength, is used to enhance the effect of the laser.

5. If the laser energy suffices, and adequate numbers of hair follicles are damaged, then the old hair growth is stunned and stopped completely after some sessions. The best results are seen when the appropriate equipment is used with the appropriate selection of recommended laser parameters for that equipment.
